As life expectancy increases due to quality healthcare and achievements in modern medicine, the number of older Americans living with chronic disease also expands. We need your help with the American Geriatrics Society’s efforts to urge Congress to support the ‘Geriatric Assessment and Chronic Care Coordination Act’ (S 1340/HR 2244).
